Any such person or persons, firm, corporation or other entity desiring such a permit shall make an application to the township setting forth his, her and/or its name and address, the names of the owners or reputed owners of the property, the address and location from which he, she and/or it is moving from and the new address or location of the place to which he, she and/or it is moving to.
