Prior to issuance of each permit, every applicant shall furnish the make, model and serial number of the vehicle to be used; and the name, address and telephone number of the owner and/or operator employed. Each applicant shall pay the sum of $10.00 for the permit for each vehicle which shall cover the period from October 1st to April 15 of the following year.
