Upon issuance of the original Restricted Weapons Owner's Identification Card and upon filing of any supplemental application thereafter, the Chief of Police shall issue a "Proof of Registration" form for each listed weapon. The Proof of Registration form shall contain the name of the person filing the application and to whom the Restricted Weapons Owner's Identification Card was issued; the date of expiration of the card; and the description of the registered item, including serial number.
