§ 99.01 CHARGES FOR AMBULANCE SERVICE.
   (A)   A person using the emergency ambulance service of the city shall be charged the sum of $950 for basic life support. For purposes of this division, “SERVICE FOR BASIC LIFE SUPPORT” means care provided to a person that could be provided by an individual certified as a basic emergency medical technician only.
   (B)   A person using the emergency ambulance service of the city at a care level of advanced life support Level 1, advanced emergency medical technician, intermediate emergency medical technician or paramedic emergency medical technician, shall be charged the sum of $1,300 per incident in lieu of any charge imposed in division (A) above. For purposes of this division, an emergency ambulance run will be considered an ALS Level 1 run under this division if advanced life support services are specifically requested or skills at an advanced level of training are actually used on the run.
   (C)   A person using the emergency ambulance service of the city at a care level of advanced life support Level 2, advanced emergency medical technician, intermediate emergency medical technician or paramedic emergency medical technician, shall be charged the sum of $1,500 per incident in lieu of any charge imposed in division (A) above. For purposes of this division, an emergency ambulance run will be considered an ALS Level 2 run under this division if advanced life support services are specifically requested or skills at an advanced level of training are actually used on the run.
   (D)   In addition to the charges imposed in divisions (A), (B), and (C) above, the person for whom services are rendered shall be charged the sum of $18 per loaded mile, or portion thereof, from point of pick up to final destination.
   (E)   A person utilizing the emergency ambulance service of the city for basic life support at the scene of an incident where the patient refuses transport to a hospital after care has been rendered shall be charged the sum of $1oo. Examples of basic life support care rendered are, but not limited to: bleeding control, fracture stabilization and basic emergency childbirth at home. Charges outlined within division (D) are not applicable for charges within this section since transportation to the hospital does not occur.
   (F)   A person utilizing the emergency ambulance service of the city for advanced life support care at the scene of an incident where the patient refuses transport to a hospital after care has remedied the emergency medical condition or in the event that emergency resuscitative methods result in field termination by the medical control physician shall be charged the sum of $150. Examples of advanced life support care rendered are, but not limited to: emergency diabetic care, seizure care and advanced cardiac life support care. Charges outlined within division (D) are not applicable for charges within this section since treatment or transportation to the hospital does not occur.
   (G)   A person utilizing the emergency ambulance service of the city for the sole purpose of lifting assistance only without the necessity of treatment nor transportation shall be charged a sum of $25 per incident occurring after three incidents within a calendar year. Charges outlined within division (D) are not applicable for charges within this section since treatment or transportation to the hospital does not occur.
   (H)   There shall be no charges imposed, in addition to those set out above, for medical supplies used by the patient during any type of ambulance service run.
   (I)   For purposes of this section, an “EMERGENCY AMBULANCE SERVICE RUN” is defined as the act of being transported from a point of pick up to a medical facility or other destination, even if the person for whom the services are requested is not actually transported. If ambulance service is used to transport a person from point of pick up to a medical facility and, then from that medical facility to another medical facility or point of pick up, such use shall be considered as two separate trips.
   (J)   A person utilizing the emergency ambulance service of the city who remits payment by check and the account is determined to have non-sufficient funds shall be charged the sum of $25.
   (K)   In addition to the charges imposed in divisions (A), (B), and (C) above, persons who are administered oxygen while receiving medical care from Union City Emergency Medical Ambulance shall be charged a sum of $20.
