§ 95.28 CONTENTS OF THE NOTICE.
   The notice to the property owner of the determination that they are responsible for an unsafe sidewalk should include the following:
   (A)   A statement that it has been determined that they are responsible for an unsafe sidewalk;
   (B)   A description of the location of the unsafe sidewalk;
   (C)   A statement that the homeowner has the right to contest the determination that they are responsible for an unsafe sidewalk by following the procedure set out in this subchapter;
   (D)   An explanation that the City of Union City may at its discretion, remove and dispose of the existing sidewalk to assist in the replacement of such unsafe sidewalk at no cost.
   (E)   A statement that if the person acknowledges responsibility for an unsafe sidewalk as defined above and applies for the opportunity to have the City of Union City remove the existing sidewalk.
   (F)   A copy of the relevant code section.
