On forms furnished by the City and available at City Hall, the collector shall furnish the following information, sign the form and certify that the form correctly states the information set forth therein. The hotel-motel tax return shall set forth:
(A) Total transactions divided into all taxable and nontaxable transactions.
(B) A substantiation of sales to governmental agencies or non-profits exempt from tax.
(C) The amount of tax due.
(D) Such other information as may be reasonably required.
