All applications for employment shall be made on forms prescribed by the City Manager or designee. Such forms may require background information to include training, experience and other pertinent information. All applications must be signed, and the City Manager or designee may require proof of statements. Application forms shall not elicit any information concerning age, race, political affiliation, sex, religion, the existence of any mental or physical disability, or national origin.
