The application for such license must contain at least all of the following information and must be properly sworn to under oath according to law.
(a) Application shall contain the purpose and/or purposes of the solicitation, canvassing, or selling and such purposes shall be set forth in detail.
(b) If the permit application is for the solicitation of funds within the City, the names of the bank or banks in Uhrichsville or elsewhere where the funds will be deposited shall be set forth therein.
(c) The application shall contain the names and addresses of all of the persons who are connected in any way whatsoever with the solicitation, canvassing or selling to be carried on, and for which the permit is requested within the City, and shall also include all persons, corporations, associations who will participate in any of the division or proceeds of such solicitation and further shall include the names and addresses of all of the solicitors, canvassers, and/or sponsors.
(d) The application shall also contain the percentage, distribution to be made of the funds collected, including the intermediate and ultimate use of all the funds so collected; the fees charged by the solicitors and/or managers of the solicitation and the names and addresses of all of the solicitors and/or managers collecting the fees.
(e) The application shall also contain a financial statement of each of the sponsors, the solicitors, salesmen, employers, and the manager or supervisor of the salesmen and/or solicitors within the City. Such financial statement shall be complete, current and not older than sixty days preceeding the date of the filing of the application.
(f) The application shall also contain the permanent address of the manufacturer, distributor, sales agency, and any and all other persons, or corporations or associations which are responsible for all of the representations made by the salesmen to the public of the products being sold.
(g) The application shall also have filed with it as a part of it any and all written contract forms or agreements or printed material which are to be used in the soliciting or selling for which the license is being applied.
(h) The application shall also contain the period of time the solicitation and/or canvassing or selling shall take place, which period shall not exceed thirty days from the date of the issuance of the license. (Ord. 1073. Passed 11-14-68.)
