711.02 LICENSE REQUIRED; FEE.
   (a)   No person being the owner of a gameroom or amusement arcade shall display for public patronage or keep for operation by the public, any mechanical or electrically operated amusement device without first obtaining a license issued by the Mayor in accordance with this chapter.
   (b)   Each owner of a gameroom or amusement arcade shall pay the sum of twenty dollars ($20.00) per machine per year for an annual license for such gameroom or amusement arcade.
   (c)   The license required herein shall not be transferable to any other person.
   (d)   The license fee paid for gameroom or amusement arcade licenses shall be prorated as follows:
      (1)   If such application is filed and the license issued prior to July 1, of any year the fee shall be one hundred dollars ($100.00).
      (2)   If such application is filed after July 1, of any year the fee shall be fifty dollars ($50.00).
   (e)   No license shall be issued until the application is submitted to the Mayor, which application shall, among other things, state:
      (1)   The name, address, age and birth date of the applicant.
      (2)   The location where the mechanical or electrical amusement devices are to be displayed or operated and the business conducted therein.
      (3)   The name of the owner or owners of the premises wherein the devices are to be displayed or operated.
      (4)   The name of the owners of the business conducted therein.
      (5)   Whether or not the applicant or the owner of the machines has been convicted of a gambling offense or a drug offense in the last ten years prior to the date of application.
      (6)   The name of the owner of the machines, if the person is different from the applicant.
   (f)   This chapter shall not apply to establishments that have been issued a liquor permit by the State, except that it shall apply to any establishment that has only the following permits:
      (1)   C-1 Low Powered beer in original containers for carry out only, and/or
      (2)   C-2 High powered beer, malt liquor and wine in original containers for carry out only.
