(a) There shall be two classes of building sewer permits as follows:
(1) For residential and commercial service; and
(2) For service to establishments producing industrial wastes.
(b) In either case, the owner or his agent shall make application on a special form furnished by the Butler County Health Department, which the applicant shall supplement by any plans, specifications and other information as are deemed necessary by the Health Officer.
(c) A permit and inspection fee according to the schedule of fees on file shall be paid at the time the application is filed.
