(A) The traffic tag shall consist of a heavy paper tag, divided into two identically serially numbered parts by a perforated line. The upper part shall contain a notice of the violation and shall be attached to the vehicle. The lower part of stub shall contain a report of the violation and shall be filed with the Traffic Clerk.
(B) The traffic tag shall be substantially in the following form:
