Any person desiring to secure a license under this chapter shall apply in writing to the city clerk on a form provided by the city, at least ten (10) days in advance of the effective date of the license. Such application shall state:
A. Name, address and telephone number of the applicant.
B. Name, address and telephone number of the person, firm or corporation by whom the applicant is employed.
C. Description, license number, year, make and model of the vehicle being utilized.
D. Nature or character of the publications, goods, wares, merchandise or services being offered by the applicant.
