(A) Requirement.
(1) No driveway shall be constructed or altered or any culvert installed without a permit authorizing said construction, alteration, or installation from the village. Construction of any driveway shall include the installation of a culvert meeting the specifications provided in this subchapter.
(2) No driveway shall be constructed or altered or culvert installed without a permit authorizing said construction, alteration, or installation from the village. All driveways shall include a culvert satisfying the specifications of this subchapter.
(B) Application. Application for a permit shall be available from the Village Hall and an application shall be made to the Building Inspector on the form provided.

(C) Fee. There shall be no fee for securing a driveway and culvert permit.
