Such application shall set out, under oath, the following information:
(a) The name under which the dealer intends to do business;
(b) The address of the yard and separate sales office, if any;
(c) Whether the business shall be conducted as an individual, co-partnership or corporation;
(d) The full name and resident address of the applicant, if an individual; the name and residence addresses of all partners, if a co-partnership; and the names and addresses of all officers, if a corporation;
(e) The names of all certified weigh-masters, if any; and
(f) A list of the equipment used in the business, such as trucks, or other vehicles, storage facilities and make and capacity of scales.
