112.02. Designated customer service areas.
   (a)   The following municipal offices are hereby designated Customer Service Areas for purposes of this policy:
      - Customer service desk of Department of Public Utilities
      - reception area of the Offices of City Council
      - Toledo City Treasurer
      - Such other areas as the Mayor may designate
   (b)   Such designated Customer Service Areas shall offer an opportunity to register to vote together with applications for services, renewal or recertification for services and change of address relating to such services.
   (c)   Such designated Customer Service Areas shall provide assistance to applicants in registering to vote, and in cases in which such designated Customer Service Areas would provide assistance with its own forms or website, such agency shall provide the same degree of assistance with regard to voter registration as is provided with regard to the completion of its own forms or website, unless the applicant declines such assistance.
   (d)   Such designated Customer Service Areas shall also receive and transmit all completed physical applications completed by Lucas County resident to the Human Relations Commission or its designee who shall forward the completed registrations to the Lucas County Board of Elections, unless the applicant requests to send the form themselves. Applicants that are not eligible to vote in Lucas County but are eligible to vote in Ohio will be required to send their completed application to the appropriate County Board of Elections.
   (e)   Provided however that this section shall not apply to services that must be provided to prevent actual or potential danger to the life, health, or safety of any individual or of the public.
