(a) The request for Board review must be filed within fourteen (14) days from receipt of the Police Division determination. The Board may waive the fourteen day limit for permitting late filings in extraordinary circumstances.
(b) A request for review must be made personally (by appointment) or filed in writing at the Safety Building on the form designed for that purpose.
(c) The request for review shall include:
(1) Name, address and telephone number of the complainant;
(2) The date the original complaint was filed with the Police Division;
(3) The substance of the original complaint and the reason for the appeal;
(4) Whether the complainant believes the Board should hear witnesses and, if so, the name, address and telephone numbers of all witnesses and;
(5) A copy of the written determination of the Police Division Internal Affairs must be attached to the form.
