   (a)   All sub-space, gratings, covers and openings shall be kept closed during all times when such openings are not actually in use and such openings shall be kept in good order and repair as determined by the Commissioner. All supporting members within such sub-space shall be kept and maintained in a safe condition as determined by the Commissioner. The surface of the street above sub-space must be maintained by the owner in a smooth, level condition, without cracks or crevices as determined by the Commissioner. Should any sub-space, or any wall, opening or support pertaining thereto, be deemed to be in disrepair, as determined by the Commissioner, the same shall be rebuilt or repaired by the owner. Owner shall follow City of Toledo building code and permitting requirements for all corrective repairs.
   (b)   The liability insurance or bond required by this chapter shall be at all times kept in full force and effect until the sub-space to which the same relates has been filled and the surface of the street there over restored as determined by the Commissioner. Failure to so maintain such bond or insurance shall, in addition to other remedies, cause the immediate revocation of the Sub-space Occupancy Permits issued for such sub-space.
