Applications for permits shall be made to the Board of Review upon blanks provided by the Director and shall set forth the name and address of the applicant, the trade name under which he, she or it does business, the class of vehicle (taxicab or motor vehicle for hire/limousine) for which the permit is desired, the seating capacity, the design, the color scheme and lettering and marks to be used on such vehicle and any other information required by the Director. Applicants for permits must meet all qualifications set forth in Toledo Municipal Code Section 775.01 for applications for City operating licenses except those listed in Section 775.01(e).
Applications for, and renewals of, a motor vehicle for hire/limousine permit must include written verification of the applicant's current registration with the Public Utilities Commission of Ohio ("PUCO") as an intrastate passenger carrier and also the applicant's current United States Department of Transportation ("USDOT") registration number. In lieu of a USDOT registration number, an applicant may provide a notarized affidavit certifying that the applicant is not subject to USDOT regulation because none of applicant's business crosses state lines. No new or renewal permit for a motor vehicle for hire/limousine shall be issued without this registration verification.
Under no circumstances shall permits be issued to an individual or company that does not reasonably expect to have the permitted vehicle on the road and being driven by the permit holder or his employee within sixty (60) days. Under no circumstances shall a permit application be granted to any applicant who currently has one or more permits under revocation for a reason other than non-use. Permits shall not be issued in anticipation of transfer to others. If the application is granted, the applicant shall furnish to the Director prior to issuance or renewal of the permit information to include the make and model, the state license number, the name and address of the owner of the vehicle, a list of all persons who will be operating the permitted vehicle and any other information required by the Director. All vehicles shall display any license plate and/or sticker as required by the State of Ohio.
