(A) The city shall conduct formal evaluations every 12 months, which shall be designed to improve the performance of the employees.
(B) The annual evaluations shall be conducted by the division head or department head and shall be reviewed by the City Manager. A standard form and forms shall be adopted for the evaluation interviews, and then shall be a permanent part of the employee's personal service record. This record shall be taken into consideration for promotions.
