§ 37.31  INSURANCE.
   (A)    The city shall provide to all non- bargaining unit employees working at least an average of 30 hours per week, a group hospitalization/major medical insurance plan for those employees electing to accept such coverage. For the base medical plan, the city shall pay 88% and the employees shall pay 12% of the premium cost. For the benefit of employee choice and reduced costs, the City Manager may offer alternate plans to the base medical plan. The percentage cost share for said plans may include a reduced employee premium contribution over the base plan above. In no case, however, shall the city's actual dollar premium contribution for said alternative plans be higher than that for the base plan. Employees shall be required to complete an authorization for payroll deduction in order to be covered by any plan.  The city shall provide a $50,000 term life insurance policy at no cost to the employee.
   (B)   The type of insurance and method of providing the above described insurance coverage, and/or the choice of insurance carrier, shall be solely within the discretion of the city.
   (C)   The city shall provide accident and disability insurance to all volunteer/part-time EMS personnel in accordance with departmental standard operating guidelines.
   (D)   Effective September 1, 2005, in lieu of receiving medical insurance benefits, employees with single or family medical coverage may elect to opt out and receive reimbursement for said waiver. The amount of said payment in lieu of insurance benefits, as well as eligibility requirements, shall be determined by the City Manager, but in no case shall the amount paid exceed $2,400 per year.
   (E)   Upon separation from employment all full time employees, employees' spouses and employees' dependent children shall be eligible for continuation of health, medical, hospitalization and dental insurance in accordance with the provision of the Consolidated Omnibus Budget Reconciliation Act of 1986 (COBRA).
