§ 150.36  APPLICATION.
   (A)   Application for a certificate of registration shall be upon the form issued by the Director of Planning or his or her designee.
   (B)   Evidence of repeated violations of the building code or other ordinances shall be sufficient evidence to disqualify the applicant from receiving a certificate of registration.
