(A) Simultaneously with the remittance subscribed in § 127.05, each telecommunications carrier shall make a written report to the Village Treasurer for the period for which the remittance applies stating as follows:
(1) The name of the telecommunications carrier.
(2) The telecommunications carrier's principal place of business.
(3) The number of network connections to which the surcharge applies.
(4) The amount of surcharge due.
(5) Such other reasonable and related information as the corporate authorities may from time to time require.
(B) The provisions of this section are effective through December 31, 2015. As of January 1, 2016 and thereafter, the provisions of Illinois Public Act 99-0006 shall apply, and as such Act may be amended from time to time thereafter.
