§ 91.31 APPLICATION; EXCEPTIONS.
   (A)    Application for a dog license shall be made upon such printed forms as shall be provided by the village and shall include the following information.
      (1)    The owner of the dog proposed to be licensed.
      (2)    The residence and telephone number of such owner.
      (3)    The name, age, sex, breed, and description of the dog proposed to be licensed.
   (B)    Each application for a dog license shall be accompanied by a certificate of rabies vaccinations signed by a veterinarian that the dog proposed to be licensed has been properly vaccinated against rabies within the preceding 12-month period.
   (C)    The provisions of this section shall not be intended to apply to dogs whose owners are non-residents of the village temporarily within the village nor to dogs brought into the village for the purpose of participating in any dog show, nor to "seeing-eye" dogs properly trained to assist blind persons when such dogs are actually being used by blind persons for the purpose of aiding them in going from place to place.
   (D)   Application for a license must be made within 30 days after obtaining a dog over three months of age. This requirement will not apply to a household who is caring for a dog or cat for no longer than 60 days.
