142.03  PERMIT APPLICATION.
   (a)   All applications for a permit under provisions of this chapter shall be submitted on a form provided by the Director of Administration.

   (b)   Information  required on the application shall include but not be limited to:
      (1)   Street address of premises where alarm system is located.
      (2)   Name of  the  principal resident, business or  organization occupying premises and the day and night telephone numbers of such.
      (3)   Name, address and  telephone  numbers of  at  least  three persons who have access to the premises at all times and who can  respond  to  assist  with the investigation of an alarm activation. Any changes in this information must be reported within seventy-two hours.
      (4)   Type of alarm system.  (If a combination alarm which has different  meanings  at  different times of day, provide full details.)
      (5)   Location  and types of all sensors and signals of the alarm system (preferably shown on a floor plan of the premises).
      (6)   Any additional information which will assist the Fire or Police Departments  in  responding  to an activation of the alarm system.
      (7)   Whether applicant requests monitoring services which means allowing the alarm to dial directly into and report police, fire and  EMS  incidents to  the City of Tiffin  Police  Dispatch Center  for  a  monthly  fee  of  $40.00.  The City at its sole discretion  may invoice the monitoring fee  on  a  monthly, quarterly, or annual basis.
