(a) No person shall tap or make connections with any sewer or cause the same to be done without first having procured a license as hereinafter provided. Any person desiring to make connections with a sewer or engage in the business of making sewer connections shall make application in writing to the City Engineer. The City Engineer shall examine the qualifications of the applicant, and if he finds that the person is qualified to make connections with the sewers, he shall grant a license to the applicant. Such license shall authorize the licensee to make connections with sewers under the provisions of the ordinances of the City and the laws of the State. If the licensee violates any of such ordinances or laws relative to the opening of the streets or the connecting of any sewer, the City Engineer shall suspend such license for a period of time he deems necessary or may revoke the license.
(b) A sewer builder's license shall not be issued until the applicant files with the City Engineer a liability bond in the sum of one thousand dollars ($1,000) with the sureties to the approval of the City Engineer, conditioned that he will obey all ordinances of the City and laws of the State relative to street openings and sewer connections, and that he will indemnify and hold harmless the City from all loss or damage that may be either directly or indirectly occasioned by making connection with any sewer or the making of the necessary excavations therefor. Should the sureties withdraw from the bond or the bond for any reason become of no effect, then the license issued shall immediately become void.
(c) A sewer builder's license shall not be issued until the applicant files with the City Engineer a Certificate of Insurance from an insurance company authorized to conduct business in Ohio, stating that the applicant has in effect Comprehensive General or Contractors Liability Insurance and Completed Operations Insurance with minimum bodily injury limits of two hundred fifty thousand dollars ($250,000) per occurrence and five hundred thousand dollars ($500,000) aggregate and property damage limit of one hundred thousand dollars ($100,000) per occurrence and aggregate. Should this insurance coverage for any reason be terminated, then the license issued shall immediately become void.

(d) A fee of twenty dollars ($20.00) shall be charged annually by the City Engineer for the issuance of an annual sewer builder's license.

(e) The bonds and licenses provided for in this section shall be in effect for the calendar year of issuance, with the calendar year commencing on January 1 and ending December 31.
