(A) In order to obtain a permit, an application shall be submitted to the Village Building Official and shall contain the following information:
(1) All requirements as set forth in the Hamilton County Driveway Regulations, as amended from time to time.
(2) Statement as to:
(a) Types of motor vehicles using the driveway;
(b) Frequency of use, estimate of average number of vehicles that will use driveway per business day;
(c) Times of day driveway will have heaviest use;
(d) Whether new driveway construction, relocation of existing driveway or change in use is intended for a parcel with an existing driveway. If a driveway already exists, is it intended to be removed;
(e) Description of current and proposed use of property;
(f) Such additional information as the Village Building Official deems relevant in evaluating whether a permit should be issued.
(3) Signed by owner or authorized representative.
(4) Drawing or plat which depicts existing and proposed driveways, location for buildings and structures and topography of existing property.
(B) The Village Building Official may waive specific information or documentation if he or she determines that due to the scope of the proposed project, the cost of providing it is not justified and all relevant and material information is otherwise available.
(C) If the use of the driveway deviates in any significant manner from the use described in the permit, the Village Building Official may revoke the permit. No further use of the driveway will then be permitted until the use conforms to the application or a modified permit is issued and the applicant completes any required modifications.

(D) A complete copy of such standards is on file with the Clerk of the Legislative Authority for inspection by the public and also on file in the County Law Library, and the Clerk has copies available to the public for distribution to the public at cost. (R.C. § 731.231)
