The application for the driver’s license shall be made upon blanks furnished by the town. The application shall give the age, home address of the applicant, whether the applicant is in sound physical condition, including good hearing and good eyesight, whether or not he uses intoxicating liquors, drugs or narcotics, and whether or not he has been convicted of or has pleaded guilty or nolo contendere to the violation of any town, state, federal, or other criminal law, and if so, the number of times and kind of offenses, and such other information as the Town Board of Commissioners shall require.
