The form of a certificate of public convenience and necessity issued under this chapter shall be prescribed by the Town Board of Commissioners, and shall contain the name and address of the owner, the number, kind, and description of vehicles, the operation of which is authorized by the certificate, and special limitations prescribed by the Town Board of Commissioners in granting the certificate, and the certificate shall state that it is issued subject to the provisions of this chapter and all other laws and ordinances governing the operation of taxicabs within the town limits.
