A Mobile Food Vehicle Operator, prior to commencing Mobile Food Vehicle operations within the City, shall furnish the Zoning Administrator with the following documents and information:
   (a)   A current, written certification that the Mobile Food Vehicle has passed all necessary inspections required by the Sylvania Township Fire Department;
   (b)   A current written certification that the Mobile Food Vehicle Operator and the Mobile Food Vehicle have passed all necessary inspections and are in full compliance with the Lucas County Health Department;
   (c)   A letter of good standing from the City Income Tax Division with respect to the Mobile Food Vehicle operations; and
   (d)   A current certificate of insurance evidencing general liability coverage in the minimum amount of one million dollars ($1,000,000.00) in the aggregate in a form acceptable to the City.
