(a) The City of Sylvania will submit annual reports to Ohio EPA. The report will include:
(b) The status of Sylvania's compliance with permit conditions, an assessment of the appropriateness of the identified BMPs, progress toward achieving the statutory goal of reducing the discharge of pollutants to the Maximum Extent Practicable (MEP), and the measurable goal for each of the minimum control measures;
(c) Results of information collected and analyzed, if any, during the reporting period, including monitoring data used to assess the success of the program at reducing the discharge of pollutants to the MEP;
(d) A summary of the storm water activities the City of Sylvnia plans to undertake during the next reporting cycle (including an implementation schedule);
(e) Proposed changes to Sylvania's Storm Water Management Plan, including any changes to any BMPs or any identified measurable goals that apply to the program elements; and
(f) Notice that the City of Sylvania is relying on another government entity to satisfy some of Sylvania's permit obligations (if applicable).
