(a) Applicants for a license to operate a game room under this chapter shall file a written, sworn application on a form provided by the Director of Public Safety, which application shall be signed by the applicant if an individual, by all partners if a partnership and by the president if a corporation. Such application shall set forth the following information: name, date of birth, social security number, nation of citizenship, location of place of business, past criminal record, the number of mechanical amusement devices to be located on the premises, together with a description of each device and other such information as may be required by the Director of Public Safety
(b) Applicants for a license to display, exhibit or maintain five or less mechanical amusement devices in any establishment other than a game room shall follow the procedure set forth in division (a) of this section. (Ord. 72-82. Passed 9-20-82.)
