Any person, including an applicant for a tattoo or body-piercing establishment permit, who engages in the practice of tattooing or body-piercing as herein defined shall file an application for a tattooing or body-piercing permit with the Village President upon a form provided by the Village President and shall pay a nonrefundable filing fee of $65.00 for an original application and $65.00 for a renewal application, to the Village Clerk, who shall issue a receipt which shall be attached to the application filed with the Village President.
