Users seeking a waste discharge permit shall complete and file with the director an application on a form prescribed by the director, accompanied by the applicable fees. The director may further require that the application include any additional information deemed necessary by the director in order to determine if pretreatment will be required to meet the conditions and limitations of this article, or to meet EPA/DEM pretreatment standards. The director may further require any information which is required by EPA/DEM.
