The application for a permit to dig any trench or make any excavation or opening or any other alteration in any of the streets, alleys, public highways, rights-of-way, or easements of the city shall be made to the city and state clearly the name of the street or highway or location of easements or rights-of-way in which it is proposed to make such opening; the purpose of making the opening; the name of the person or entity who proposes to make such excavation or opening; and any other information required under the provisions of this subchapter.
