The appeal shall be perfected by directing written request for same to the Director within 30 days of the issuance of the decision, interpretation or ruling providing therein the following information:
(A) The name and address of the person making the appeal;
(B) The facts giving rise to the particular ruling or refusal to make a ruling;
(C) The ruling or refusal to make a ruling appealed from; and
(D) The reasons why the ruling should be set aside or, if the ruling was refused, the reasons why the ruling should be made.
