(a) Co-payments. Each Village of Sunbury employee or other covered family member participating in the Village of Sunbury Health Insurance Benefits Program, excluding elected officials, shall be subject to co-payments as required by the approved plan.

(b) Incentives for Non-participation.
(1) Village employees who have elected not to participate in the Village of Sunbury's employer-provided health insurance benefits program commencing prior to January 1, 2019, are hereby offered the following incentives by calendar year for non- participation:

NOTE: Only employees enrolled in the employer provided health insurance benefits non-participation program prior to 01/01/2019 are eligible for the non-participation incentive set forth above.
(2) Incentive shall be paid on a monthly basis for the month not covered.
