139.30 HEALTH INSURANCE COVERAGE.
   Group hospitalization and medical coverage is available to eligible employees. Coverage for qualified dependents is also available. COS will contribute a portion of the premium costs for such Plan. Employees must also contribute a portion of the premium costs for such Plan
   Coverage for health will begin on the first day of the month following the completion of sixty (60) calendar days of full time employment.
   Health benefits coverage terminates after all accrued leave is exhausted. The employee would then be eligible for COBRA benefits.
   (a)    Any employee that is terminated for gross misconduct will not be eligible to COBRA benefits.
   Gross misconduct can include but is not limited to use or possession of controlled substances, alcohol, assault upon a co-worker, sexual harassment, sabotage, serious breaches of health and safety rules, theft of time, misuse of City owned property, including computers and cell phones. All COS employees are expected to conduct themselves in a professional manner while on duty.
