139.21 VACATION.
   Paid vacation leave is provided to all eligible full-time employees.
   COS shall provide graduated paid vacation time to eligible employees based upon their most recent length of service with the City. Earned vacation credits shall be calculated for eligible employees according to the following table:

   The maximum accumulated is three hundred eighty-four (384) hours during the fiscal year (July 1 - June 30). Any accumulation upon termination is paid at the hourly rate in effect on the day of termination from employment.
   Vacations shall be scheduled consistent with the needs and efficient operation of the City. All vacations must be approved by the supervisor. In addition, all vacation in excess of three (3) days must have thirty (30) days in advance approval. This requirement may be waived in certain situations as approved by your supervisor or the
Mayor.
   When an employee is scheduled to return to work on his or her next regular shift after his or her vacation, he or she will be charged a vacation day every day he or she is absent (this includes Saturday or Sunday if that is the employee's next regular shift). There will be a maximum of thirty (30) consecutive vacation days unless approved by the Mayor.
   Minimum Allowable Vacation
   Vacation periods taken shall be in a minimum of one (1) hour increments.
   
   Vacation Changes
   Changes in vacation dates may occur at the request of an employee provided that such change does not hamper the efficient operation of his or her department. Request for change must be submitted in writing at least three (3) working days prior to the anticipated date.
   
   Vacation Credit Carry Over
   Up to 240 hours vacation credit may be carried over for use into the following year as determined on June 30. Any amount over 240 hours must be used by the employee prior to June 30; or will be forfeited.
