(a) The application for a permit to operate a massage establishment shall set forth the exact nature of the massage to be administered, the proposed place of business and facilities therefor, and the name and address of the person in control of and responsible for the operation of the establishment.
(b) In addition to the foregoing, any applicant for a massage establishment permit shall furnish copies of the massage therapy licenses issued by the Ohio State Medical Board pertaining to each person who will provide massage services at the establishment.
