Application for a permit required by Section 901.01 shall be made at the office of the Director of Public Service on forms provided by him. The application shall show:
(a) The name and address of the owner or agent in charge of the property abutting the proposed work area;
(b) The name and address of the party doing the work;
(c) The location of the work area;
(d) The estimated cost of the alteration; and
(e) Such other information as the Director finds necessary.
