703.10 RECORDS OF CALLS REQUIRED.
   (a)    Upon receipt of an emergency call, the private provider/operator shall notify the Stow Police/Fire Dispatch Center for a Stow Fire Department emergency medical service response and shall keep records of the call in accordance with subsections (b) and (c) below.
   (b)   A report, in triplicate, shall be made on every emergency call for every patient, stating the emergency medical technician's assessment and impressions, and detailing the treatment and care that was rendered at the scene and during transport of the patient. A copy shall be left at the hospital emergency room, a copy shall be kept with the ambulance company's permanent records.
   (c)   The report shall also contain the following:
      (1)   The time the call was received.
      (2)   The time the emergency medical service vehicle was dispatched.
      (3)   The time the emergency medical service vehicle arrived at the location of the patient.
      (4)   The time the emergency medical service vehicle left for the hospital.
      (5)   The time the emergency medical service vehicle arrived at the hospital.
      (6)   The time the emergency medical service vehicle was again available for service.
      (7)   The name of the hospital.
      (8)   The name and address of the patient, except as provided in subsection (a) above.
      (9)   The name of the driver and the emergency medical technicians.
