The application form supplied by the City shall contain at least the following information.
(a) The applicant shall complete, swear to, sign and have notarized an application supplied by the City and filed with the appropriate City department.
(b) An agreement by the applicant that he assumes the duty of dispatching any available emergency medical service vehicle driver and technician to each emergency call, and that all drivers or EMTs sent out on an emergency call shall undertake to render first aid or treatment to the patient when necessary.
(c) An agreement by the applicant that upon receipt of an emergency call, he shall immediately notify the Police Department, if an emergency medical service vehicle, driver, and technician are not then available for immediate dispatch and response within a reasonable period of time.
(d) A statement of the applicant's employees' certification by the State, of training and experience of himself and of his drivers and emergency medical technicians in the care and transportation of patients.
(e) An agreement by the applicant that the City shall not be liable for the payment of ambulance service fees incurred by the initiation of a call by the City or its Police or Fire Departments on behalf of any person or persons not in the custody or care of the City.
(f) An agreement by the applicant that in the event of a catastrophic situation or of simultaneous emergency situations existing outside of the City, the Safety Director or his designated representative has the authority to require one licensee, in the Safety Director's discretion and upon his direction, to maintain one ambulance on call for emergencies in the City to respond to emergency calls for so long as the situations prevail.
(g) A statement by the applicant that the service is an approved provider under guidelines established by the State Department of Public Welfare and Medicare, with the ability to render statements to patients or third parties for reimbursement purposes.
(h) An agreement that the applicant shall have emergency medical technicians, drivers, ambulances or emergency medical service vehicles readily available twenty-four hours a day, seven days a week.
(i) Any other information as the Safety Director or his designated representative may request under the advice of the Commission which is reasonably necessary for compliance with this chapter.
