(a) The Safety Director or his designated representative may issue a license to a licensee/operator of an ambulance or emergency medical service vehicle using the streets of the City if the following requirements are met.
(1) The application form supplied by the City Safety Director's office shall be completed, sworn to and signed by the license applicant; the application form shall be notarized and submitted to the Safety Director or his designated representative for action. No licensee/operator shall submit more than one application form for each emergency medical service vehicle.
(2) No licensee/operator shall use a name on an ambulance or emergency medical service vehicle or advertise under any name other than the name in which the license is issued.
(3) The licensee/operator shall employ only drivers or emergency medical technicians who fulfill the requirements as stated in Section 703.02.
(4) Each emergency medical services vehicle shall be equipped with a siren, emergency flashing lights, trade name or business painted on each side of the vehicle and any other reasonable markings as may be prescribed by the Safety Director or his designated representative, with the advice of the Public Safety Service Commission.
(5) While in service, ambulances, emergency medical services vehicles, and equipment ordered after January 1, 1979, shall comply with standards established by the State Department of Health, including modifications to these standards promulgated by that agency. At that time, however, as the State Emergency Medical Services Advisory Council promulgates ambulance and equipment standards and any standards are enacted into law, these ambulance and equipment standards shall thereafter replace the State Department of Health Standards.
(6) All ambulance services must possess operating capability for direct radio communications with their headquarters and with hospital emergency departments. The hospital emergency department will serve as medical control for paramedic runs. Furthermore, each ambulance shall have the capability to rapidly communicate with the Police Department either by direct communications or other dispatch.
(7) Each emergency medical service vehicle, its equipment and garage shall be inspected by the Safety Director, or his designated representative, the Police Chief and the Fire Chief, or an authorized representative for compliance with such safety and health requirements as may be set forth in this chapter and by the advice of the Public Safety Services Commission. Each emergency medical service or ambulance service shall have adequate office space. Sleeping quarters shall be provided for working shifts greater than twelve hours in duration, in accordance with requirements of the Occupational Safety and Health Act. Each emergency medical service vehicle must have certification of the Ohio Department of Health, when such certification process becomes available.
(8) The applicant shall submit to the Director of Public Safety, signed certificate of insurance complying with the insurance requirements of the Ohio Division of Ambulance Licensing, as well as certification as required by the City of Stow, for the Bureau of Workers Compensation, in accordance with the Ohio R.C. 4123.01.
(9) Each emergency medical service vehicle, when not in service for patient care or otherwise in operation, shall be kept in a heated garage.
(10) The application shall disclose the individual names of owners, managers and controllers of the emergency medical service vehicle or ambulance business.
(11) Each applicant shall have emergency technicians, drivers, ambulances or emergency medical service vehicles readily available twenty-four hours a day, seven days a week.
(b) The Safety Director or his designated representative may grant the license, if satisfied as to the safety of the emergency medical service vehicles, the equipment provided, the character of the applicant, and compliance with the regulations of this chapter and other regulations promulgated by the State under Ohio R.C. Chapter 3303 and the Public Safety Services Commission.
(c) Nothing in this Code shall be construed to require an operator of a wheelchair transport/ambulette service to obtain a license prior to operating within the City of Stow.
