65.11   DOCUMENTS NEEDED FOR SEPTIC CONSTRUCTION PERMIT.
   1.   Environmental Health's application form with payment.
   2.   Professional Evaluator's site evaluation and design diagram if applicable.
   3.   Maintenance contract if required.
   4.   Easement rights if required (easements shall be recorded with the Story County Recorder).
