§ 115.22 APPLICATION FOR PERMITS; CONTENTS.
   (A)   A written application for a permit to solicit shall be subscribed and sworn to by the president or other chief officer of the not-for-profit organization making the application and filed with the Village Clerk 15 days before the time when a permit is desired.
   (B)   The application, which shall be referred to the Village Board of Trustees, shall contain the following information:
      (1)   Name of the not-for-profit organization applying for a permit to solicit and the address of its headquarters;
      (2)   Names and addresses of its principal officers and management;
      (3)   The purpose for which the solicitation is to be made and the use or disposition to be made of any receipts;
      (4)   The tax identification number issued by the Internal Revenue Service classifying the status of the application as not-for-profit and exempt;
      (5)   The name of the person by whom the receipts of the solicitation shall be disbursed and, if there are any participating organizations, the names and employer identification numbers of the participating organizations and the names of officers and employees disbursing the receipts for the participating organizations;
      (6)   The name and address of the person who will be in direct charge of conducting the solicitation;
      (7)   An outline of the method to be used in conducting the solicitation;
      (8)   The time when the solicitations shall be made, giving the proposed dates for the beginning and ending of the solicitation;
      (9)   A statement of the character and extent of the charitable, religious, educational or philanthropic work being done by the application organization; and
      (10)   A statement representing to the village that no solicitation shall be conducted or attempted at any residence, office, business or establishment or upon any individual who makes it known, by sign, statement or otherwise that no solicitation is invited.
