§ 115.02 CERTIFICATE OF REGISTRATION REQUIRED.
   Every person desiring to engage in peddling or soliciting from persons in residences within the village is hereby required to make written application for a license to peddle or solicit, which shall include the following.
   (A)   An application for a license to peddle or solicit shall be filed with the Village Clerk. The application shall truthfully state in full the information requested, to wit:
      (1)   The name and address of the present place of residence and length of residence at the address, business address and Social Security number;
      (2)   Address and place of residence during the past three years if other than the present address;
      (3)   Name and address of the person, firm, corporation or association the applicant is employed by or represents and the length of time of the employment or representation;
      (4)   Description sufficient for identification of the goods, wares and merchandise the applicant desires and intends to sell;
      (5)   The period of time for which the license is applied for;
      (6)   Whether a license issued to the applicant under this subchapter has ever been revoked;
      (7)   Whether the applicant has ever been convicted of a violation of any of the provisions of this subchapter or an ordinance of any other Illinois municipality regulating soliciting;
      (8)   Whether the applicant has ever been convicted of the commission of a felony under the laws of this or any other state or law of the United States; and
      (9)   Additional information as the Village Clerk or Board of Trustees may deem necessary to process the application.
   (B)   All statements made by the applicant upon the application or in connection therewith shall be under oath.
   (C)   The Clerk shall cause to be kept an accurate record of every application received and acted upon, together with all other information and data pertaining thereto and all licenses issued under the provision of this subchapter and of the denial of applications.
