(A) Applications for permits required by § 110.022 shall be made in writing to the Village Clerk. Each application shall state thereon the name of the owner of the premises, the name of the person or corporation constructing the billboard and specifications showing the size, material and manner of construction of the billboard.
(B) Application for a billboard permit shall be made to the Village Clerk upon a form provided by the Clerk and shall be accompanied by information as may be required to assure compliance with all appropriate laws and regulations of the village, including:
(1) Name and address of the owner of the billboard;
(2) Name and address of the owner or person in possession of the premises where the billboard is located or to be located;
(3) Clear and legible drawings with a description definitely showing the location of the billboard which is the subject of the permit and all other existing billboards whose construction requires permits when the billboards are on the same premises; and
(4) Drawings showing the dimensions, construction supports, sizes, electrical wiring and components, materials of the billboards and method of attachment and character of the structural members to which the attachment is to be made. The design, quality, materials and loading shall conform to the requirements of the Building Code and other portions of this code effective at the time of application. If required by the Clerk, engineering data shall be supplied on plans certified by an engineer licensed to practice in the state.
