8-13-6-4: PERIODIC COMPLIANCE REPORTS:
   A.   Any user that is required to have an industrial waste discharge permit and performs self-monitoring shall comply with all applicable requirements under 40 CFR 403.12 and submit to the Village during the months of June and December, unless required on other dates or more frequently by the Village, a report indicating the nature of the effluent over the previous reporting period. The frequency of monitoring shall be as prescribed within the industrial waste discharge permit. At a minimum, users shall sample their discharge at least twice per year. In cases where the pretreatment standard requires compliance with a best management practice or pollution prevention alternative, the user must submit documentation required by the Village or the pretreatment standard necessary to determine the compliance status of the user.
   B.   The report shall include a record of the concentrations (and mass if specified in the wastewater discharge permit) of the pollutants listed in the wastewater discharge permit that were measured and a record of all flow measurements (average and maximum) taken at the designated sampling locations and shall also include any additional information required by this chapter or the wastewater discharge permit. Production data shall be reported if required by the individual wastewater discharge permit or applicable categorical standards. Both daily maximum and average concentration (or mass, where required) shall be reported. If a user sampled and analyzed more frequently than what was required by the Village or by this chapter, using methodologies in 40 CFR Part 136 and the most recent edition of Standard Methods for the Examination of Water and Wastewater, it must submit all results of sampling and analysis of the discharge during the reporting period.
   C.   Significant non-categorical industrial users must submit to the Village at least once every six (6) months a description of the nature, concentration, and flow of the pollutants required to be reported by the Village. The reports must be based on sampling and analysis performed in the period covered by the report, and in accordance with the techniques described in 40 CFR Part 136. Any user with a discharge not subject to categorical pretreatment standards must perform appropriate reporting under 40 CFR 403.12.
   D.   All periodic compliance reports must be signed and certified in accordance with subsection 8-13-6-12A of this chapter.
   E.   All wastewater samples must be representative of the user's discharge. Wastewater monitoring and flow measurement facilities shall be properly operated, kept clean, and maintained in good working order at all times. The failure of a user to keep its monitoring facility in good working order shall not be grounds for the user to claim that sample results are unrepresentative of its discharge.
   F.   The Village may require reporting by users that are not required to have an individual wastewater discharge permit if information or data is needed to establish a sewer charge, determine the treatability of the effluent, or determine any other factor which is related to the operation and maintenance of the sewer system.
