Applications for licenses shall be in writing and shall state under oath:
(A) The name and address of the applicant and the names and addresses of any partners or other persons associated in the management of the dance hall;
(B) The address and description of the premises and the name and address of the owner if other than the applicant;
(C) The hours when dancing will be allowed on the premises.
