§ 114.06 APPLICATION.
   Application for a license must be made at least ten working days before the regulated activity is proposed to begin and must include an accurate, sworn statement in writing, on a form furnished by the city, that gives the information listed below and includes the required supporting documentation:
   (A)   Full legal name;
   (B)   Complete home and local address of the applicant;
   (C)   A brief description of the proposed activity, its location, and the merchandise or service involved;
   (D)   The date and hours of the day during which the activity will be conducted;
   (E)   Name, address, and phone number of the person on whose behalf the orders are solicited, together with credentials establishing the applicant's relationship to the person;
   (F)   Whether or not the applicant has been convicted of any crime other than petty traffic violations; the nature, time and location of the offense; and the punishment or penalty imposed;
   (G)   The last cities or other localities, not exceeding five, where the applicant conducted the proposed activity immediately preceding the date of the application and where within those areas the activity took place;
   (H)   Make, model, year, color, and state license number of each motor vehicle to be used in connection with the proposed activity;
   (I)   Social security number, date of birth, and driver's license or state or tribal identification card number;
   (J)   Passport information and visa status if the applicant is not a United States citizen;
   (K)   The name, address, and phone number of the person or company employing the individual, if different from the information in division (E) above;
   (L)   A copy of all documents to be used by the applicant in the regulated activity;
   (M)   Phone number(s) for the applicant; and
   (N)   Mobile food unit additional requirements.
      (1)   Valid copies of all necessary licenses or permits required by the state or county;
      (2)   Pictures of the mobile food unit vehicle and a site plan showing location of the vehicle on the property. Plans and specifications for the non-vehicular mobile food stand or unit and a site plan showing the proposed location of the stand or unit on the property;
      (3)   A copy of the written permission from the property owner or property owner's agent where the mobile food vendor will be conducting business;
      (4)   A copy of the written permission of any restaurant within 150 feet of the mobile food unit conducting business;
      (5)   A certificate of insurance; and
      (6)   A list of other communities or locations where the applicant has been a mobile food vendor.
